Kokoro is Disc 2, Track 20 on the Xenosaga Original Soundtrack, Disc 1, Track 18 on the Xenosaga Episode I album, and Track 2 on the Kokoro / Joanne Hogg Theme from "Xenosaga Episode I" album. It was also included as track 7 on the album Colours of Light (a compilation of Mitsuda's vocal works).
An instrumental version was provided as part of the Kokoro / Joanne Hogg Theme from "Xenosaga Episode I" album, and the track was remixed as Omoide - Kokoro for the Across the Kosmos Jazz in Kosmos album. The piece itself is a vocal remix of Omoide.
It plays in the final cutscene of the game when Shion sees KOS-MOS standing on the bow of the Elsa as they descend into Second Miltia, and continues through the credits.

ヴォーカルのジョアンヌも凄く気に入ってくれて、唄い 方も情感たっぷりといった感じでした。 シンプルな楽曲 なのにも関わらず“グッ” と来ます。 やっぱり歌ものはヴ ォーカルの力が楽曲の良し悪しを決めてしまいますね。

Official translation
I aimed to create a continuous flow with a gradual build-up until the end. At the beginning, I mimic the guitar's rhythmic drive in the left hand with an ostinato-like pattern, setting the foundation for the piece's evolving intensity.
