Kokoro is Disc 2, Track 20 on the Xenosaga Original Soundtrack, Disc 1, Track 18 on the Xenosaga Episode I album, and Track 2 on the Kokoro / Joanne Hogg Theme from "Xenosaga Episode I" album. It was also included as track 7 on the album Colours of Light (a compilation of Mitsuda's vocal works). An instrumental version was provided as part of the Kokoro / Joanne Hogg Theme from "Xenosaga Episode I" album. It is a vocal remix of Reminiscence.

It plays in the final cutscene of the game when Shion sees KOS-MOS standing on the bow of the Elsa as they descend into Second Miltia, and continues through the credits.
