Two Worlds and Two Hearts is Disc 9, Track 13 on the Xenoblade Chronicles 3 Original Soundtrack and Disc 20, Track 13 on the Xenoblade Chronicles Original Soundtrack Trinity Box.
It is an orchestral version of At Our Life's End and Future Awaits.
It plays in the final cutscene of Future Redeemed before the credits, where Matthew says his last words to N, and later leaves the nascent new City.

エンディングでマシューとの別れの時に流れる曲ですが、 別れの寂しさと未来を切り開く前向きな気持ちが入り 交じった曲になります。 タイトルにある「二人の想い」はマシューとナエルのことだけに焦点を当てているわけで はなく、これまでの色々なキャラクターにも当てはめて聞く事ができるのではないかと思い、あえてこのような タイトルにしてみました。 この世界が明るい未来になると予想し、曲の最後はメジャー (明るい) コードで終わっ ています。
